How do I apply for Permission for Delayed Birth Registration?

Permission for Delayed Birth Registrationis a citizen-centric e-District service; citizen can apply for Permission for Delayed Birth Registration through any of the following ways-

• Visiting state service delivery portal www.assam.gov.in
• Visiting Public Felicitation Centres available at DC Office Dhemaji, SDO (Civil) Jonai, Revenue Circle Office Dhemaji/Gogamukh/Sissiborgaon/Jonai
• Visiting Common Service Centres available throughout the district

Citizens are required to submit the following supporting documents along with thePermission for Delayed Birth Registration application form-

1. Scan copy of application form
2. Register Hospital Govt. / Pvt. Certificate regarding Birth or Age Proof  (any)
3. School Certificate/Admit Card (for age 6 and above) or parent's details
4. Affidavit
5. Any other document
